Records Requests

The Carlstadt Volunteer Ambulance Corps maintains patient records in accordance with NJ Law. Due to HIPPA regulations, patient records are not subject to Open Public Records Act requests.  Requests for record will be honored subject to receiving a proper request. Requests must include the following information.

1. Patient’s Name
2. Patient’s Address
3. Patient’s Date of Birth
4. Incident Date
5. Incident Location
6. Name & address of 3rd party authorized to receive the records (such as your attorney).

A government issued photo ID of the requestor showing the signature must accompany the request.

Records will also be released based on a court order, subpoena, or warrant.
